Our goal is to help clinical care teams provide more efficient, effective, and family-centered care that optimizes quality of life and well-being for children and youth with I/DD and their families. This position will serve as a professional peer resource for families seen by primary care providers (PCPs) in partner practices and will provide education, training, and guidance to practice staff on how to effectively support children with IDD and their families, including connection to local community resources.

This position will serve as a family system navigator (FN). This position works directly with patient families to provide emotional, informational, and instrumental support to acquire desired services, resources, supports, and therapies. The FN duties and roles include and are not limited to the following: Supporting families (with emphasis on Eastern NC/Trillium connected families) directly in their I/DD and Mental Health (MH) system navigation needs, work with relevant referring UNC pediatric medical providers, liaison and develop key partners in the relevant Managed Care Organizations and other key agencies such as NC START, area provider agencies (Easter Seals, ARC, ASNC, etc.), support the teaching of other UNC pediatricians and pediatric resident physicians, medical students, and LEND trainees about the I/DD system and common I/DD support needs, connect and/or network with similar efforts such as Duke Pediatrics program, and interface with other family system navigators from related projects at UNC, Duke, and elsewhere, assist in the ongoing development of FN training materials, assist in the tracking and monitoring and reporting of key outcomes. Additional responsibilities include: conduct needs assessment with patients and family to identify needed resources, communicate identified needs with clinical teams, build and provide tailored support for clinical teams, patients and families, initiate and respond to contact from families and community service providers, and help patients, families, and providers increase their knowledge of programs and systems as well as develop their advocacy skills. FNs may also receive and track referrals and document outcomes of support provided, demonstrate accurate, clear, professional, and timely communication, and engage in regularly scheduled team meetings, peer support, and supervision. FNs will document or communicate in Epic electronic health record, Microsoft Word, Microsoft Excel and Outlook email. FNs will adhere to all HIPAA compliance and patient confidentiality rules. FN's will engage in at least weekly clinical team meetings in which they will report back progress, share new information gathered from the family, and advocate on behalf of the family's self-determined and expressed priorities.

This hybrid position is part time, approximately 20 hours/week. There is some flexibility in work hours for this position, however, applicants will be expected to complete approximately 15 hours/week of their works tasks during business hours, at the identified pediatric/family care office. The remainder of the hours can be completed virtually, with at least two mandatory meetings/week that will occur during regular business hours.

Associate's degree in a related Human Services discipline and two years of experience related to the area of assignment, or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.

Physical ability to move from vehicle to clinic settings when needed. Physical ability to use, read, and manipulate laptop computers. Ability to communicate by phone, video and written communication with families, clinical team and family support team. Duties to be performed with or without reasonable accommodation.
