The purpose of this position is to provide care coordination for MCN infusion patients reviewing their medical history, diagnostics, lab, creating and maintaining a complete patient profile. This position is also responsible for updating and maintaining submissions to the National Amyloidosis Registry. This position is a .8 FTE working Tuesday- Friday 7:30 am- 4 pm and the first 4 weeks will be onsite in Golden Valley for training and then move to remote work. Provide Patient Care Coordination and Infusion Treatment Support Within the RN Scope of Practice
- Coordinate infusion services to ensure safe, timely, and effective patient care.
- Review treatment plans, provider documentation, laboratory results, imaging requirements, allergies, and medication orders prior to infusion scheduling and treatment.
- Facilitate dose calculations and modifications by assessing patient status, reviewing protocols, consulting providers, and documenting changes as appropriate.
- Monitor and coordinate infusion-related requirements including laboratory testing, imaging, therapy plan updates, medication authorizations, and follow-up appointments.
- Maintain accurate and timely documentation within the electronic medical record in compliance with HIPAA standards.
- Serve as a liaison between providers, infusion nursing staff, pharmacy, prior authorization teams, schedulers, and patients to ensure continuity of care.
- Manage infusion scheduling workflows, patient visit summaries, treatment plan updates, medication-specific monitoring requirements, and infusion preparation activities.
- Calculate and maintain medication dosing and weight-based treatment requirements for infusion therapies including IVIG and other specialty medications.
- Communicate with patients, caregivers, providers, and support staff regarding treatment coordination, follow-up needs, and infusion readiness.
Continuous Improvement and Operational Support
- Perform miscellaneous duties as requested by leadership, providers, and support staff.
- Participate in practice improvement initiatives by identifying workflow enhancements, implementing process changes, and supporting operational efficacies.
- Maintain organized tracking systems, scheduling resources, and operational documentation to support infusion services across multiple clinic locations.
- Assist with inventory-related activities, medication utilization tracking, and program reporting as assigned
- Reference and adhere to departmental policies, procedures, SOPs, and regulatory requirements.
- Collaborate with interdisciplinary teams to promote compassionate, personalized, and high-quality patient care.
Education and Experience:
- A.S. or B.S.N. degree in nursing with basic knowledge of neurology
- At least 3 years of experience in nursing; neurology experience desired.
- MN Licensure as registered nurse.
