Reporting to the Correctional Behavioral Health Supervisor (Program Director); this position is responsible for daily delivery of behavioral health evaluation and intervention services within the Department of Medicine. This position primarily provides services to patients served through the DOM Division of Correctional Medicine however may provide similar services as a part of behavioral health efforts across the Department of Medicine.

Duties & Responsibilities:
  • Interviews and assesses the needs of individuals via telehealth, phone screening, face-to-face contact, and other methods utilizing established program assessment tools and procedures.
  • Develops patient individualized plans of care, monitor progress, and coordinate care with patient providers (internal and external)
  • Provides intervention services, when needed, following program protocols and operating within scope of training - including, but not limited to, providing psychoeducation, therapy, or responding to patient crisis.
  • Acts as liaison between the institution and the individuals to be served; submits documentation to state agencies, partner organizations, and other funding sources on behalf of the program as needed.
  • Collaborates with other professional staff members in meeting the individual needs of program participants; as necessary, consults with higher-level care providers and physicians.
  • Prepares and maintains necessary records and reports for compliance, reviews, and amendments on an as-needed basis.
  • Engages in other behavioral health-related activities associated with program requirements (i.e., identifying and assisting in the coordination of needs to address patient care gaps)
  • Completes, reviews, and submits medical documentation for the purpose of documenting services, billing, and determining eligibility for new and continued services.
  • Develops resource materials and protocols for the program and department when needed.
  • Assists in overseeing and training support staff and student employees as assigned.
  • Perform other related duties and participate in special projects as assigned.


Minimum Qualifications Required:

  1. Master's degree from an accredited school or university in social or behavioral science or a related health specialty area. Based on institutional requirements for each position:
  2. Current State of IL Licensure as a Licensed Social Worker or a Licensed Clinical Social Worker.


Preferred Qualifications:
  • Experience working with justice-involved individuals.
