Hourly Pay Range:

$40.45 - $62.70 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position:

  • RN Patient Care Navigator, Cardiac Cath Lab

Position Highlights:

  • Hours: Day Shift, Monday - Friday
  • Location: Glenbrook Hospital, Glenview, IL

What You Will Do:

  • Provide coordination and navigation for patients scheduled for cardiac catheterization, structural heart, and electrophysiology procedures.
  • Serve as the primary point of contact for patients to ensure all pre-procedure requirements, testing, and education are completed.
  • Collaborate with nurse practitioners, schedulers, and the Cath/EP team to ensure seamless communication and accurate scheduling.
  • Review medical records and gather necessary information to prevent delays in procedures.
  • Contact patients via phone, email, or NorthShore Connect to deliver pre-procedure instructions and answer questions.
  • Work the scheduling queue to ensure timely outreach-within 2 weeks for standard cases and 4 weeks for complex cases.
  • Manage time efficiently to promote quality and continuity of care.
  • Participate in quality initiatives and compliance activities, including accreditation readiness and special projects (e.g., STEMI Door-to-Balloon tracking).
  • Support data collection, outcomes reporting, and continuous improvement initiatives.
  • Maintain knowledge of policies, procedures, and safety standards relevant to patient navigation and cardiac care.

What You Need:

  • Education: Bachelor of Science in Nursing (BSN) required.
  • Licensure: Current RN license and CPR certification required.
  • Experience: 1-2 years of critical care or cardiovascular nursing experience required; prior nurse navigator experience preferred.
  • Strong interpersonal, written, and collaboration skills.
  • Proficiency in EPIC or similar electronic medical record systems.
  • Ability to manage multiple priorities and communicate effectively with patients and clinical teams.
  • Commitment to patient-centered care and continuous improvement.
