Physical Therapist - (PRN - hours as needed)

GENERAL SUMMARY:

The Physical Therapist is responsible for identifying patient care needs and priorities for care through evaluation, planning, providing and/or delegating care for patients with physical and/or cognitive disabilities to improve functional independence. This position focuses on patient/family needs and assessing the patient's response to care and progress toward outcomes.



PRINCIPAL JOB FUNCTIONS:

2. Participates in meetings, committees and lean projects as assigned.

3. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

4. *Performs service excellence must-haves to achieve an excellent patient/customer experience.

5. Uses quality improvement processes, programs or outcome to help improve department operations.

6. *Performs and documents an initial patient examination and evaluates the data to identify problems and establishes and documents a plan of care with functional goals based on evaluation of the examination data and patient needs.

7. *Provides or delegates and supervises physical therapy patient intervention in a manner consistent with the examination data, the evaluation, and the plan of care.

8. *Provides and documents patient/family education regarding current condition and plan of care, including home program/discharge instructions.

9. *Performs and documents patient reexamination, reevaluates the data, and modifies and documents the plan of care.

10. *Communicates relevant patient care information with other health care professionals.

11. Maintains current clinical competency per department guidelines.

12. Assists with development and presentation of department community education programs.

13. Uses Performance Improvement principles to help improve department operations.

14. Performs other related duties and projects as assigned.

EDUCATION AND EXPERIENCE:

Graduation from an APTA accredited school of Physical Therapy required. Current Physical Therapy license with the State of Nebraska required.



OTHER CREDENTIALS / CERTIFICATIONS:

Current Basic Life Support (BLS) Certification required.
