THE POSITION

Do you have a passion for helping others? Schuylkill County Area Agency on Aging (AAA) is eager to welcome a compassionate and organized Aging Care Manager 2 to our dedicated team. In this role, you will have the opportunity to assist senior citizens find the services and support needed to stay active and safe, while remaining in the community setting of their choice. DESCRIPTION OF WORK

The Care Manager receives case assignments involving general casework, intake and assessment and works with older adults from initial contact with individuals, their families and professionals through ongoing care management responsibilities. This position assists older adults and families with completion of program forms and services. Responsibilities include specialized duties such as maintaining a caseload of consumers and regularly coordinating services with physicians, community agencies, financial institutions, hospitals, nursing homes, and personal care homes. You will be expected to respond to client emergencies, and protective services. This employee manages multiple tasks, paying close attention to detail, which includes completion of paperwork, data input of confidential client information into state databases, work assignments, and meeting deadlines. Training and team support will be provided with some training out of county and requiring overnight stay.

Work Schedule and Additional Information:

  • Full-time employment, 37.5 hours per week
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
  • Periodic, on-call hours when required after training
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Schuylkill County. If no eligible candidates who live within Schuylkill County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.

  • You must pass a background investigation.
  • A conditional offer of employment will require a medical examination and a drug screening.
  • This position falls under the provisions of the Older Adult Protective Services Act.
    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
