Location: Calmont Operations BuildingDepartment: Clinical Support & PreventionShift: First Shift (United States of America)Standard Weekly Hours: 40Summary: Responsible for performing a wide range of activities for Star Kids members to positively affect individual health outcomes. The CHW Service Coordinator will monitor a group of level 3 members who do not meet the requirements for an LMSW or RN named service coordinator. Activities include, but are not limited to outreach, assessment, Member education, service coordination and Member support.Additional Information: Responsible for performing a wide range of activities for Star Kids members to positively affect individual health outcomes. Certification/Licensure:
