Social Worker- Internal Medicine

Park Nicollet is looking to hire a Social Worker to join our Primary Care team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary:

Provides direct social work services to patients, extending services to family members in need of identified social worker services, providing them with psychosocial and environmental support. Makes available a full range of treatment modalities with individuals, families, and groups, based on social work assessment and diagnosis. Coordinates social work services with other departments and providers within the Park Nicollet Health Services i.e., hospice, hospital and clinic based social workers and out of system providers. Provides social work services to the patient and associated community. May provide clinical supervision to other Social Workers. Works with physicians, nurse clinicians, other medical/surgical specialists, and other Park Nicollet Health Services employees. Contact with patients and their families.

Work Schedule:

Monday-Friday, 8am-4:30pm

Required Qualifications:

• Master's degree in social work.
• Must be licensed as Licensed Independent Clinical Social Worker (LICSW) or LIS
• Current license to practice social work in the state of Minnesota.
