Medical Outreach Case Manager - Access to Care, Emergency Dept.

This role will focus on assisting the uninsured / under-insured with access to benefits and resource referrals in the Emergency Department, working with providers, nurses and residents.

Responsible for eligibility screenings and connections to coverage, and resources to address social drivers of health with an overall goal of decreasing utilization of the Emergency Department.

Ideal candidates may have previous experience:


  • Working directly in medical settings

  • Familiarity with state and federal programs


Full Time, 40hrs

Summary:
The Case Manager - Medical Outreach role is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients. They work on-going with patient caseloads, delving deeper into more medically and socially challenging issues and health needs, addressing barriers for patients.

Required Minimum Knowledge, Skills, and Abilities (KSAs)


  1. Education: Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field preferred.
  2. License/Certifications: Current applicable state(s) license as a Licensed Social Worker preferred. Valid driver's license and reliable vehicle required.
  3. Experience: With a bachelor's degree: Two years of experience in a community health setting, including one year of case management experience required. In lieu of bachelor's degree: Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred.
