Overview
The Pharmacist is responsible for preparing and dispensing of medications (including sterile admixtures and non-admixtures), and all related equipment and supplies in accordance with physician orders, URAC and ACHC standards, federal and state laws and regulations, and Reliance Rx policies and procedures. Must ensure that all required documentation is complete and that all products meet stability, compatibility and practice standards. Must evaluate physician ordered medications for appropriateness of therapy, and maintenance of patient specific optimal therapy through clinical management of medications provided in relation to individual diagnosis. Clinical responsibilities include but are not limited to preparation of patient records, maintaining a medication profile, care planning, evaluation of drug interactions, preparing progress/communication notes, clinical monitoring, pharmacokinetic monitoring, renal dosing, laboratory and culture monitoring, determining medication administration compliance and effectiveness, and discharge planning. Must provide drug information, perform clinical consultation and patient counseling, and communicate with physicians, nurses and patients. Must also perform ordering/management of medications, controlled substances, equipment and supplies as necessary or assigned. Professional working relationships with pharmacy staff and others must be maintained in order to coordinate clinical team activity appropriate to providing patient care.
Qualifications
Bachelor's degree required; licensed to practice pharmacy in the state of New York required. Five (5) years of pharmacy experience required. Experience in a specialty pharmacy setting and knowledge of specialty pharmaceuticals/sterile product preparation and/or the clinical components of infusion therapy and patient care preferred. Demonstration of leadership/mentoring skills in a practice setting preferred. Must be able to work independently in a motivated self-directed manner and maintain a positive attitude. Must maintain a current New York Pharmacist License. Proven examples of displaying the Reliance values: Collaborative, Accessible, Results-Oriented, Empowering, and Supportive.
Essential Accountabilities
Provide and coordinate pharmaceutical services. Perform pharmaceutical care activities for assigned patient population. Monitor, evaluate, document, and report patient outcomes. Provide or coordinate preparation and dispensing of medication, and all related equipment and supplies, in accordance with physician orders, URAC and ACHC standards, federal and state laws and regulations, and Reliance Rx policies and procedures. Perform work under general direction. Coordinate all clinical pharmacy care for assigned patient population. Evaluate all physician ordered medications for appropriateness of therapy, and maintenance of patient specific optimal therapy through clinical management of medications provided in relation to individual diagnosis. This includes but is not limited to evaluation for drug interactions, clinical monitoring, laboratory and culture monitoring, medication administration compliance and effectiveness. Provide drug information, perform clinical consultation and patient counseling; communicate with physicians, nurses and patients. Perform tasks and documentation including but not limited to preparation of patient records, patient assessment, care planning, progress/communication notes, medication profile, pharmacokinetic monitoring, renal dosing, clinical case conferences, ongoing clinical assessment, discharge planning/summary, supply management, evaluation and selection/preparation/dispensing of medications and sterile products, supplies, and equipment as needed as directed. Ensure adherence to state/federal laws and regulations, Reliance RX policies and procedures, and URAC and ACHC standards. Participate in Quality Assurance and Performance Improvement activities. Prepare/supervise preparation of sterile admixtures and non-admixtures as needed, and ensure that all required documentation is complete, and that all product meets stability, compatibility, and practice standards. Assist with procurement/management of medications, controlled substances, equipment, and supplies as necessary or assigned. Maintain professional working relationships with pharmacy staff and others in order to coordinate clinical team activity appropriate to providing patient care. Interact with other departments, attend meetings and conduct presentations as assigned. Exhibit peer leadership by maintaining pharmacy coverage and assisting other personnel in completing workload as required to maintain clinical pharmacy services and production services.

Hiring Compensation Range: $110,000 - $125,000 annually Compensation may vary based on factors including but not limited to skills, education, location and experience. In addition to base compensation, associates may be eligible for a scorecard incentive, full range of benefits and generous paid time off. The base salary range is subject to change and may be modified in the future.
