Responsibilities Coordinate the referral to admission process for home health patients Gather all pertinent referral information and enter in computer Receive physician orders Verify insurance benefits Provide excellent customer service support to referral sources, patients and team members Communicate appropriate information to team members regarding patient's needs for equipment, supplies, medications and plan of care.
