Department:Social WorkDepartment's Website: Qualifications: Key Responsibilities: Referral Intake & Client Support * Receive and review referrals for the 100 Families Initiative. * Screen referrals for appropriateness and completeness using established criteria. * Assign referrals to Family Advocates based on caseload, availability, and family needs. * Serve as first contact for referred families and individuals; provide information, support, and resource referrals as needed. * Follow up with clients regularly to ensure engagement and to identify additional service needs. Team & Administrative Support: * Assist the 100 Families Coordinator in maintaining and updating email distribution lists for alliance meetings and communications. * Draft and send meeting invitations, reminders, and follow-up communications using email and calendar platforms (e.g., Outlook, Google Calendar). * Monitor the Coordinator's calendar, help manage scheduling and send out calendar invitations as needed. * Support Family Advocates and the Coordinator with documentation, logistics, and administrative tasks as assigned. * Ensure accurate records are maintained in program systems, including data entry and tracking of referral assignments. Collaboration & Communication: * Coordinate with community partners and referring agencies to ensure timely responses to referrals. * Attend alliance meetings and internal team meetings as needed, take notes and assist with coordination. * Contribute to a positive, team-oriented environment that supports collaboration and the mission of the 100 Families Initiative. Additional Information: Preferred Skills: * Bilingual (Spanish/English or Marshallese/English) is a plus. * Familiarity with the 100 Families Initiative or similar collaborative, multi-agency models. Work Environment: This position may involve a combination of in-office and remote work, with occasional travel to meetings or partner locations as needed.
