Job Overview:

The RWJ Rahway Community Health Navigator for Chronic Disease will ensure patient access to healthcare and community services by interacting with Rahway s patients with chronic health conditions to address barriers to care, provide guidance and healthcare education, and assist in the coordination of care in order to maintain a healthy lifestyle and meet identified health goals. This role acts as a liaison between patients, healthcare providers, and community resources to ensure continuity of care and improved health outcomes.

Qualifications:

Required:

High school graduate or equivalent required

Basic computer skills required

Valid driver s license required

Annual competency assessment and goal review required

Successful completion of a hospital approved Community Health Worker, or comparable, formal training program is expected upon hire

Preferred:

Electronic medical record (EMR) experience preferred

Knowledge of medical terminology preferred

Scheduling Requirements:

8:00am/8:30am to 4:30pm/5:00pm

Monday-Friday

Essential Functions:

  • Work with hospital care teams to identify, evaluate, schedule and ensure timely patient access to appropriate health resources and services.
  • Meet, establish trust and serve as the primary point of contact for patients with chronic conditions, such as diabetes, hypertension, COPD, CKD or heart disease, in the hospital setting including the ED, hospital units and/or other setting where patients are located to ensure a smooth transition of care for patients with the highest risk.
  • Collaborate with other major disease navigators, such as oncology, to ensure patient care is coordinated across the continuum for all DRG s.
  • Conduct initial and follow-up assessment to identify and connect patients with clinical services both at RWJUH Rahway and across the RWJBH system of providers. Provide social and community resources to patients as needs are identified
  • Assist patients with resources needed to understand their health condition(s), access screenings, adhere to medical advice and medications, and develop strategies to improve their health and well-being
  • Assist patients with resources to recognize identified social issues; substance abuse, food insecurity, transportation, childcare services, housing and homelessness and any other issues that are impacting the members lives
  • Participate in quality improvement and population health initiatives related to chronic disease management.
  • Track and document patient progress, outcomes, and interventions in accordance with HIPAA and organizational standards, no later than 24 hours after patient contact
  • Maintain knowledge of, and relationships with, community-based organizations. Keep current a directory of regional/local referral sources
  • Participation in interdisciplinary rounds, department meetings, safety huddles and other meetings as required
  • Perform daily data entry. Collect and report data to hospital leadership as requested
