Social Worker- Full-time-TEMP position- (3-month contract)- Queens, NY

Job Purpose:

FlexStaff is seeking a Social Worker on behalf of our client. Under the direction of the Social Work Manager, the Temporary Social Worker will provide essential social work services to participants, advocating for their needs and assisting with various aspects of their lives, including psychological, financial, environmental, and interpersonal matters. This role will engage with participants in diverse settings, including their homes, PACE sites, and community locations such as hospitals and skilled nursing facilities. The Social Worker will act as a liaison among participants, authorized representatives, the Interdisciplinary Care Team, and other community agencies to ensure the highest level of satisfaction throughout the PACE lifecycle.

This is a TEMP 3 Month Contract Role

Key Responsibilities:

- Advocate for and provide ongoing social work services to participants and their authorized representatives to assist them in understanding care delivery recommendations and addressing personal and environmental challenges.

- Educate participants on community health services, benefits, and available resources, helping them navigate systems like SNAP, Medicaid, and Medicare.

- Ensure timely communication and coordination of care during hospitalizations and assist with discharge planning and follow-up visits.

- Conduct biopsychosocial assessments and mental status evaluations; facilitate end-of-life discussions and coordinate palliative care services.

- Maintain accurate and timely documentation using EMR systems, ensuring compliance with standards and regulations.

- Collaborate with the IDT to develop and implement participant-centered care plans and facilitate psycho-educational presentations.

Qualifications:

- Master of Social Work (M.S.W.) from an accredited college or university.

- Minimum of one (1) year of experience in a healthcare setting, particularly with a frail or elderly population. Experience in home care or fieldwork is preferred.

- Valid NY State Social Work license (LMSW or LCSW) is required.

- Regular access to a car for potential field visits is preferred.

- Strong interpersonal, communication, and organizational skills.

- Ability to manage workload efficiently and demonstrate case management and clinical skills.

- Participation in continuing education and professional training is encouraged.
