Key Responsibilities:
*Serve as a core member of the care team, coordinating healthcare services within the facility and referrals to external providers. *Conduct patient assessments and collaborate with families to develop personalized care plans. *Provide education and support to patients and families to help them navigate healthcare options. *Work closely with third-party payers to obtain necessary authorizations and ensure coverage for services. *Partner with service providers, families, and patients to ensure continuity of care and access to community resources. *Maintain detailed reports and records to track patient progress and treatment plans. *Participate in case reviews with medical directors to discuss challenging cases and determine best treatment approaches.
