Overview
AbbeHealth Services - is a regional nonprofit organization dedicated to providing mental health and aging services that help individuals achieve their optimum levels of independence and success. The Abbe Center for Community Mental Health is a subsidiary of AbbeHealth Services located in Cedar Rapids, Iowa. Community Based Recovery Services (CBRS) Team Leader will be responsible for developing and implementing services that provide skill training, supportive and/or treatment for individuals in recovery from a serious mental illness. This includes involvement in the provision of services following the guidelines for Home Based Habilitation, Day Habilitation Programming, Community Support Services, Intensive Psychiatric Rehabilitation, Program for Assistance in Transition from Homelessness, Peer Led Services, Program for Assertive Community Treatment, etc. that are generally funded through Medicaid, the Region, private pay and/or grants.
Responsibilities
- Welcomes individuals and their natural supports into services and fosters professional, person centered relationships that promote strengths, resiliency, recovery, and hope. Instructs and supports team members to do the same.
- Role model the use of person first language and verbalizes respect for team members, participating individuals and their families.
- Provides administrative and clinical supervision to team members. Monitors quality of care and associated documentation.
- Ensures that Abbe Center Policies, Procedures and Practices are followed in accordance with Chapter 24 Accreditation standards and other applicable regulatory standards as required by law, funding sources, best practices, etc.
- Works with intake specialist to ensure pre-screening of all referrals to determine if eligibility criteria are met. Positively interfaces with intake specialist, referral sources, potential clients and their family members through the referral and admission processes as indicated.
- Ensures the timely and accurate completion of Admission, Intake, Comprehensive Assessment, and other paperwork that result in the development of a Service Plan based on the individuals' needs and desires, using incremental steps and measurable goals as required by updated accreditation, utilization management guidelines of funders, best practice models and accepted industry standards.
- Oversees the implementation of services and interventions outlined in the Service Plan to include (but are not limited to) skills training, support and treatment in various life areas including living, learning, work, and socialization. Ensures the timely and accurate completion of Progress Notes and other paperwork that reflects said interventions.
- Provides and encourages interventions that recognize the complex nature of individuals' resources, strengths, and needs.
- Recognizes and teaches team members of the early symptoms of physical and mental illness, and performs/coaches' staff to perform appropriate interventions and/or referrals in accordance of an individualized crisis plan. Effectively notifies other supervisors, treatment team members, guardians and other identified people of crisis or potential crisis situations. Completes Incident Reports as required by specific service lines.
- Is available for after hours, on call services as assigned.
- Ensures the recognition of the therapeutic actions and side effects of medications and reporting to prescriber.
- Lends knowledge of specialized training, education, or job-related life experience to other team members. Facilitates team members with different specialties to do the same.
- Obtains and maintains funding required to access needed services. Track funding authorizations and insurance eligibility to promote successful billing processes.
- Collaborates and partners with other treatment team members, including natural supports as desired by the individual receiving the service. Facilitates and/or attends treatment team meetings that reinforce the role of the individual as the director of the service plan and promote integrated care. Reinforces these behaviors amongst team members.
- Implements the use of groups to encourage learning from others in recovery from serious mental illness and promote peer-based advocacy as allowed by funders and specific service lines.
- Educates client and staff on client rights, responsibilities and encourages autonomous decision making consistent with identified cultural influences.
- Schedules, attends, and facilitates regularly scheduled staff meetings, supervisory sessions, and in-service trainings. Participates in management trainings and other management meetings as assigned.
- Participates in community networking and collaborates with agency partners for the good of the community and the agency as assigned. Acts as a spokesperson for assigned service lines within the Abbe Center service line.
- Collects and reports job related data as assigned.
- Participates in continuous quality improvement and corporate compliance activities to support service development based on outcomes measures, satisfaction surveys and focus groups. Supports the annual, agency CQI plan via a variety of utilization review activities including but not limited to chart audits. Works with the team on the continuous program development to best meet the evolving needs of clients and staff as well as changing expectations of funders, regulators, etc.
- Strives to implement evidenced based practices of specific service lines with high level of fidelity.
- Oversees the judicial use of available resources to meet client, staff, program, and agency needs.
- Performs fair hiring practices. Provides coaching and at least annual performance appraisals. Assists in the development of professional development goals. Documents supervisory activities.
- Assumes other duties as assigned.
Qualifications
Education Master's in the mental health field, including but not limited to nursing, social work, mental health counseling, psychiatric rehabilitation, or psychology Qualifications/Experience Prior experience providing mental health services and/or substance use disorder services preferred. Valid Iowa Driver's License Must carry current automobile insurance in accordance with Abbe's policies. Must present a recent (before first date of employment) MVR accounting from the Motor Vehicle Department, State of Iowa to the Human Resources Department. The results of this MVR must meet the requirements for violations as set forth in Abbe's motor vehicle policies.
