Position Highlights

The Division of Pulmonary, Allergy, Critical Care, and Sleep medicine, in the Department of Medicine, is seeking a Community Health Worker (Health Education and Promotion Professional I) for Dr. Sairam Parthasarathy's AZ CEAL project. The Community Health Worker is responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The incumbent supports providers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family. The Community Health Worker provides social support and informal counseling, advocates for individuals and community health needs. The incumbent must be bilingual in Spanish and English and familiar with the ability to communicate medical terminology in both languages and have a reliable form of transportation to travel to various physicians' offices within Tucson. This position may be eligible for a flexible schedule work arrangement in conversation with the supervisor.

Duties & Responsibilities
  • Assist with scheduling appointments and providing information about care options to improve patient access.
  • Support patient-provider communication by handling basic inquiries and sending reminders as directed.
  • Distribute disease management and health promotion materials based on established guidelines.
  • Participate in local healthcare facility visits and community events to help with logistics and distribute educational materials.
  • Assist in the promotion of healthy living, eating, and vaccinations by supporting outreach efforts and distributing information.
  • Assist patients/participants with consenting, completion of questionnaires, data entry, and other study procedures as needed.
  • Remotely, telephonically, and in person assist the patient/participant by assessing and identifying goals to address the social determinant issues through an IVR platform.
  • Provide appropriate resources to assigned participant using knowledge of the local community resources available based upon the participant's residential area.

Knowledge, Skills, and Abilities

  • Knowledge of local community resources.
Minimum Qualifications
  • Bachelor's degree or equivalent advanced learning attained through experience required.
  • One (1) year of relevant work experience is required.
  • Must be bilingual in Spanish and English and familiar with ability to communicate medical terminology in both languages.
  • Must have a valid driver's license and ability to drive according to the University Fleet Safety Policy
Preferred Qualifications

  • Posses a Community Health Worker certification.
  • Has working experience using REDCap.
  • Willing to receive any required certifications (ie. GCP).
