Position Highlights |
The Division of Infectious Diseases in the Department of Medicine, College of Medicine - Tucson is seeking applicants for a Care Management Coordinator (Care Coordinator III) to drive and deliver patient care and support services. The Care Management Coordinator works independently and as an integral part of the clinical team providing support services that assess and facilitate the acquisition of medical, psychosocial, and financial services needs that allow successful patient health outcomes. This position may be eligible for a hybrid work week flexible work arrangement in conversation with the supervisor. Duties & Responsibilities |
- Navigates complex patient care needs that are strategic in nature, varied, and involve evaluation and interpretation.
- Uses extensive knowledge of the Ryan White Program, creativity, and innovation in case management to strategically identify and secure funding and resource acquisition, navigate the complicated healthcare system, and resolve complex insurance issues.
- Manages and coordinates a caseload that requires a higher level of organizational skills with the ability to multi-task, and track patient needs around eligibility and social determinants of health from intake through discharge from the program.
- Manages accurate and compliant documentation of activities in designated databases, adhering to ethical guidelines, state regulations, and internal policies.
- Maintains the integrity, usability, and confidentiality of data, with a particular focus on Electronic Medical Records (EMR), to support seamless patient care. EMR documentation must be precise, as it is reviewed by internal and external clinical providers and funders for auditing purposes.
- Prepares and manages regular reports assessing the effectiveness of care management activities, which contribute to the program's broader quality management initiatives.
- Conducts initial and ongoing in-clinic assessments as required by clinical guidelines and federal and state funders.
- Formulates and facilitates individualized comprehensive care plans that integrate life-saving medical and social services, as well as treatment adherence support.
- Maintains effective communication with the multidisciplinary team and external partners as that is paramount to drive outcomes and care.
- Educates and councils patients on risk reduction and healthy behaviors amongst other topics that facilitate needs being met and quality of life.
- Serves as patient advocate and program representative with outside entities like insurance carriers, foundations, medical providers, social services, state program coordinators etc., that facilitate patient needs and ensure funded service objectives and goals are met.
- Establishes relationships, collaborations and serves as a liaison between stakeholders, healthcare providers and patients.
- Provides guidance and support to junior staff within the area of expertise.

Minimum Qualifications |
- Bachelor's degree or equivalent advanced learning attained through professional level experience required.
- Minimum of five (5) years of relevant work experience, or equivalent combination of education and work experience.
