Case Manager Opportunity

Coastal Harbor Health System is comprised of a 145-bed hospital for children and adolescents, providing both short- and long-term inpatient care, and a 50-bed inpatient hospital serving men and women, ages 18 and up. The Case Manager coordinates non-therapy needs for residents, including case management, staffing, discharge planning, and utilization review, by coordinating with Therapists and other departments regarding residents' needs. This includes communicating on a regular basis with external contacts, including referral sources, state-appointed guardians, and other agencies related to residents' care.

Job Duties/Responsibilities:

  • Complete initial chart reviews and patient handbooks with newly-admitted residents; communicate with therapists to anticipate and report any foreseeable problems regarding the continued stay or discharge planning of residents
  • Document all external and internal contacts and relay patient information according to guidelines; maintain ongoing list of referral sources and aftercare providers for residents
  • Coordinate with therapists, physicians, and other treatment team members to ensure that referral sources, aftercare providers, and other involved parties receive appropriate communication
  • Attend treatment team meetings for assigned residents to provide and collect information pertinent to the progress, needs, and discharge planning of residents; complete reviews of charts and communication with unit staff and therapists to provide utilization review information to Utilization Review department; coordinate discharge planning and aftercare arrangements

Qualifications

Requirements:

  • Master's Degree in Social Work, Psychology, or other Mental Health field is preferred; a Bachelor's Degree with relevant experience may be substituted
  • Experience in Mental Health field is preferred
  • Minimum of one year related experience
