The Medical Group Consulting RN is a member for the interdisciplinary team who is the pivotal person responsible for identifying the physical, psychological, social, and spiritual needs of those patients and families needing assistance during normal business hours of the primary care clinics. Primary Responsibilities: * Assesses the patient's current health status and recommends appropriate level of care based on clinical judgement and evidence-based protocols. * Adheres to ethical, legal, regulatory and accreditation standards. * Assess and addresses clinical questions related plan of care via telephone or MyChart * Manages utilization through education. * Assesses consumer's understanding of their condition and identifies potential care and/or provider gaps. * Provides ongoing education to patients regarding their current health conditions. * Provides post-acute follow-up via telephone. * Assists in clinical quality initiatives. * Perform other duties as assigned or requested.
