ROLE SUMMARY: The Home Health Triage/ Scheduling Coordinator LPN works with members of the home health team to triage patient care needs, efficiently evaluates patient care needs and scheduling of patients to home health team members and works as part of an interdisciplinary team to provide high-quality home health services to our patients.

ESSENTIAL RESPONSIBILITIES WITHIN GUIDELINES of WASHINGTON STATE LPN SCOPE OF PRACTICE:

  • Functions as resource for nursing unit.
  • Proactively assesses, particularly new staff's stress level and ability to manage patient assignments and offers assistance as needed.
  • Coordinates orientee, volunteer and student assignments with appropriate unit staff.
  • Assist the Patient Services Coordinator in assigning patient care to nursing and therapy staff to assure appropriate staffing levels and productivity.
  • Monitors, tracks, and facilitates patient admissions, interdisciplinary care issues, discharges, hospital/Skilled Nursing Facility transfers and resumptions of care.
  • Serves as a resource for the clinical team in medication reconciliation, Home Health guidelines interpretation, order entry, medical record retrieval, and care coordination.
  • Participates with Patient Access/Intake team and scheduling in prioritizing referrals, calling patients/families, and performing consults as needed.
  • Reports pertinent information to Nurse Manager regarding the Home Health operations for that day.
  • Participates in leader rounding.
  • Promotes team building and acceptance of change for continual improvement of patient services.
  • Mentors and educates staff members, students, and others as needed.
  • Participates in assigned meetings and committees.
  • Monitors unit's quality assurance activities, such as core measures, fall prevention, as well as other unit-based priorities.
  • Participates in accreditation surveys and the maintenance of regulatory standards.
  • Facilitates problem solving, conflict resolution, and accesses the chain of command as needed.
  • Participates in after-hours Home Health and Hospice call coverage including evening, night, weekend, and holiday call coverage.
  • Maintains knowledge of home visit practices and provides patient care visits as needed.
  • This role does not current qualify for remote duty with the exception of call coverage.
  • Generally expected to function in this role when on duty.

QUALIFICATIONS AND REQUIREMENTS

Education: Completion of LPN nursing program and unrestricted license as an LPN within the state of Washington.

Experience: Minimum of one (1) year Hospice, Home Health, or Palliative Care experience preferred

Licenses/Certificates/Registration: Current WA state driver's license and proof of insurance.

Additional Requirements: Cell phone for use during working hours; car for travel to and from home visits during shift.

FTE: 1.0_, Shift: Days_, Schedule: _Days; including some weekends and holidays. Participation in the Home Health call coverage rotation required.

Requirements

  • Current Licensed Practical Nurse in Washington State or Multistate license endorsement (MLS)
  • Current BLS certification
  • License to pass medications may be required
  • Previous experience as an LPN, in an acute care setting within the last three (3) years preferred
  • Experience in assigned specialty, or successful completion of a specialty focused residency post hire
  • Successful completion of appropriate competencies for assigned specialty

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $35.15 - $50.60 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
