Work Schedule:

80% FTE, The scheduled shifts are Monday - Friday, between 8:00 am to 5:00 pm. No weekends, nights, or holidays. When onsite, you will work at 7974 UW Health Court, Middleton, or a UW Health Primary Care Clinic, location to be determined.

We are seeking a Registered Nurse to:



  • Join a freshly redesigned RN Care Coordination team which supports adult patients living with chronic conditions.
  • Connect patients with resources to help them manage their health condition(s) and achieve healthier lives. Communicate with patients/families via telephone and electronic medical record, providing both your triage and care coordination skills.
  • Collaborate with an interdisciplinary team of physicians, advanced practice providers, social workers, schedulers and other nurses to care for patients enrolled in the program.

Qualifications

  • Graduate of school of nursing Required
  • Bachelor's Degree in Nursing or higher Preferred

Work Experience

  • 6 months RN experience Required
  • Previous experience in adult acute care or ambulatory setting Preferred
  • Previous Case management experience Preferred

Licenses & Certifications

  • Registration as a registered nurse in state where employed or licensure in the state licensure compact Upon Hire Required
  • CPR/BLS Upon Hire Required
  • Case Management Certification Preferred
