The Academic Patient Centered Team Care (A-PCTC) clinic is an innovative Detroit-based patient-centered medical home for medically and socially complex adult patients and families. Its dual mission is to advance the health and well-being of our community and to educate tomorrow's healthcare leaders via clinical training programs, research, and quality improvement. Internal medicine faculty physicians and residents lead interdisciplinary care teams. The teams seek to address each patient's medical, behavioral, and social needs. The A-PCTC clinic's daily work is focused on improving the patient experience, teamwork, quality of care, and education. It is a fast-paced, team-oriented environment looking for motivated individuals who are highly adaptable to change and want to improve urban healthcare delivery.

GENERAL SUMMARY:

Provides leadership in the coordination and delivery of quality compassionate patient care. Utilizes the nursing process of assessment, planning, implementation, and evaluation to provide, delegate, supervise and document care and teaching of patients and family. Effectively communicates with the health care team. Demonstrates clinical competence. May require a professional certification which is a regulatory requirement in the State of Michigan or a documented community standard for the position, works independently and in collaboration with principal investigator when appropriate.

EDUCATION/EXPERIENCE REQUIRED:

Graduate from an accredited school of nursing. Bachelor's Degree preferred. Successful completion of Henry Ford Ambulatory orientation. Maintains and demonstrates current clinical knowledge in area of specialty. Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans. Ability to perform a wide variety of professional nursing services. Ability to effectively communicate with patients, families, and medical group personnel. Preferred experience in the specialty. Basic computer knowledge and keyboarding skills.

CERTIFICATIONS/LICENSURES REQUIRED:

Current licensure by the Michigan State Board of Nursing as a Registered Nurse. Current BLS or ACLS per Henry Ford Health policy. Certification will be required when it is a regulatory requirement in the State of Michigan or a documented community standard, i.e., Oncology Certified Nurse (OCN).
