Overview

Under the direction of the Wound Care Center Clinical Nurse Manager/Clinical Coordinator, the Registered Nurse (RN) primarily provides patient care and handles intake and discharge processes for the Wound Care Center patients as scheduled. The position may also serve as a Case Manager to a group of assigned Wound Care Center patients, as needed.


Responsibilities

Clinical Excellence/Patient Care

  • Provides patient care to Wound Care Center patients and handles patient intake and discharge processes as assigned and according to the hospital and nursing standards.
  • Develops individual plans of care using a collaborative approach with the patient/family and health care team.
  • Promptly completes documentation to assure an accurate legal record of patient's care.
  • Delivers high quality care utilizing the nursing process while demonstrating knowledge and clinical skills of the specialty required on assigned unit in accordance with established policies, procedures and standards of care.
  • Utilizes holistic approach (considering biopsychosocial, spiritual, educational, safety, and related criteria) appropriate to the age of the patients and families served in the assigned service area.
  • Delegates and provides direction for other healthcare providers and holds them accountable for their involvement in the plan of care.
  • Utilizes critical thinking in making independent judgments and maintains responsibility and accountability for the knowledge of conditions of assigned patients.
  • May also serve as Case Manager to a group of patients as assigned. This entails performing the initial nursing assessment, communicating findings with the assigned physician, assessing the wound stage and measurements, documenting care plans, evaluating the patient's status in regard to desired outcomes, and identifying patient care situations that require intervention.
  • Participates in quality improvement initiatives, providing education to the patient or caregiver, and serving as a patient advocate in the delivery and coordination of the patient care
  • Performs hyperbaric oxygen therapy (HBO)-related duties as assigned, upon completion of the Healogics or other approved hyperbaric training.
  • Collaborates as needed with other healthcare providers, Wound Care Center Providers, the Program Director and the Medical Director regarding clinic and patient needs.

Development & Patient Education

  • Continues professional development and involvement in professional activities that enhance nursing practice by sharing knowledge through networking and research.
  • Assesses and chooses teaching strategies appropriate to the patient's and their families' age specific needs, culture, gender, readiness to learn and level of comprehension.
  • Consults appropriate resources within the hospital to provide specialized education.
  • Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.

Qualifications

Education:

* Graduate of accredited nursing program (Must obtain BSN within 5 years of hire.)

* Bachelor of Science in Nursing (BSN)

Experience:

* One (1) year of clinical experience in hospital or SNF or LTACH setting.

* Wound Care experience

License(s)/Certification(s):

* Current licensure in good standing to practice as a Registered Nurse in Iowa.

* Must possess and maintain current Basic Life Support (BLS) certification.

* Valid driver's license when driving any vehicle for work-related reasons.

Medical Surgical, Home Health or Wound Care Certification preferred

Knowledge/Skills/Abilities:

* Writes, reads, comprehends and speaks fluent English.

* Multicultural sensitivity.

* Microsoft Office - basic computer skills.

* Customer/patient focused.

* Critical thinking skills using independent judgment in making decisions.

Other:

* Satisfactory completion of Finley Nurse Residency program with 1 year of start date for all nurses with minimal or no prior acute care experience.

* State of Iowa Mandatory Reporter Training possessed or completed within 6 months of hire.

* Use of usual and customary equipment used to perform essential functions of the position.

* Note: Work may occasionally require travel to other UPH facilities. May drive a UPH vehicle, rental or personal vehicle.
